Another testing failure costs millions. Skype has stated that it's recent system crash was due to a Windows security update that was incompatible with their VoIP software.
I really think Skype needs to be held accountable for not testing all software that is installed on their production servers, and not just passing on the blame to a Windows security patch problem.
If I had anything to say about it, those production servers would remain totally isolated from any software updates until, any and all patches could be tested. And I don't just mean whether the system appeared functional.
We all know that memory leaks, and extraordinary load can be introduced in software patches and those could delay failure for days, but fail they will.
It seems that an there is a major testing failure of one type or another every day or two, so the question is how many are we going to take before we insist that companies we deal with test their software.
